Under the general supervision of the Intervention Services Supervisor, the crisis worker is responsible for the direct provision of services to the general public for the primary purpose of preventing disabilities, promoting mental and physical health, and improving social and community functioning. This includes on-call crisis coverage as it relates to Chapters 51, 54, and 55 providing telephone and mobile crisis intervention, crisis counseling, assessment, and referrals through a rotation of daytime, evening, and weekend hours. The crisis worker also provides short term service coordination and monitoring of services to a specific group of individuals diagnosed with a developmental disability, emotional disturbance, substance use disorder, and/or chronic mental illness. The case manager is expected to work on a highly independent basis and to work cooperatively with community providers, law enforcement, school personnel, clients, and families.

Position Summary:   -Facilitate, compile and collect information for a detailed assessment and case plan on each in individual served.-Complete a long-term care function screen on OH program participants.-Coordinate planning and long-term goals for individuals in conjunction with families and other agencies.-Compile and assemble necessary information to complete various financial forms and applications required for individual's benefit.-Document ongoing monitoring of services within individual files.-Travel to and coordinate home visits for individuals on caseload to monitor progress of services.-Assists individuals and families in locating prospective living situations specific to the person's individual needs, in the least restrictive environment. -Facilitate and initiate correspondence with other agencies and service providers through written communication, and/or oral communication.-Coordinate and develop discharge plans for individuals in patient treatment facilities by working cooperatively with local hospitals. -Compile and report necessary information for case management billing purposes, HSRS, CIP reporting.-Know and understand Targeted Case Management, CIP Waiver, and COP rules, program guidelines and eligibility guidelines. -Provides financial management to case specific individuals including budgeting, monitoring individual checkbook and general individual accounts.

Qualifications:   -Bachelor's Degree in Social Work, Psychology, Rehabilitation, Special Education or related field.-A 30-minute mobile response time is required while performing on call duties.
-One to two years related experience and/or training; or equivalent combination of education and experience.
-Requires a valid Wisconsin driver's license and adequate insurance as required by County Policy.
-Requires Social Work licensure from State of Wisconsin or ability to obtain one.
