Salary:
The starting salary is $48,588 per year, plus excellent benefits. The pay schedule/range is 11/09. A 6-month probationary period is required. This position is represented by the Service Employees International Union (SEIU) bargaining unit.

The Department of Health Services (DHS); Division of Long Term Care (DLTC); Southern Wisconsin Center in Union Grove has multiple vacancies.  This announcement may be used to fill future vacancies.

Job Duties:   This position coordinates the development of comprehensive functional assessments and individual program plans; monitors and integrates resident programs to ensure continuous active treatment; conducts special staff training; monitors all aspects of the residents Plan of Care; and ensures staff appropriately address areas of concern by documenting and monitoring documentation of resident treatments and preparing related reports.

Special Notes:   


Due to the nature of the position, all applicants who may be appointed to this position will be required to allow the Department of Health Services to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.


Special Requirements: Must be a Qualified Mental Retardation Professional (QMRP).  This is a professional who has at least one year experience working directly with persons with mental retardation or other developmental disabilities and is one of the following: (1) a doctor of medicine or osteopathy; (2) a registered nurse; or (3) an individual who holds at least a Bachelor's degree in a professional category specified in the Federal Register as meeting the QMRP qualifications as outlined in 483.430 of the Code of Federal Regulations.

Job Knowledge, Skills and Abilities:   Knowledge of interdisciplinary team process and staff training; developmental model and principles of normalization; Intermediate Care Facility/Mental Retardation (ICF/MR) requirements for active treatment; rights of developmentally disabled persons under State and Federal regulations; monitoring, recording and report writing; center organization and appropriate communication links; ability to communicate effectively orally and in written form; and ability to assist in restraining residents in crises situations.
